GLOWDOCS Medspa VIP Membership – Comprehensive Terms & Conditions
1. MEMBERSHIP OVERVIEW & FEES
The GLOWDOCS Medspa VIP Membership (“Membership”) is a recurring monthly program designed to provide Members with access to discounted services, exclusive benefits, and priority scheduling. By enrolling, the Member agrees to all policies, payment requirements, medical consents, and conditions stated herein.
1.1 Membership Benefits
Members receive the treatments, injectable units, and/or service credits listed on the GLOWDOCS Medspa Membership sign-up page at the time of enrollment. GLOWDOCS Medspa reserves the right to revise or enhance benefits as needed for business or clinical reasons.
1.2 Activation Fee
A one-time, non-refundable activation fee of $50 is charged immediately upon enrollment. This fee covers account setup, membership onboarding, administrative handling, and access initiation.
1.3 Monthly Fees
Membership fees are automatically billed monthly to the payment method on file. All fees are charged in advance, prior to receiving services for the applicable month. Fees are non-refundable except where required by law.
1.4 Member Eligibility
To participate, individuals must:
– Be 18 years or older.
– Complete all intake, consent, and medical history forms.
– Be medically cleared for services covered by the Membership.
GLOWDOCS Medspa may decline Membership enrollment for any clinical or business reason.
2. INCLUDED SERVICES, ROLLOVER POLICY & DISCOUNTS
2.1 Included Monthly Services
Members receive the specific treatments, injectable units, and/or credits listed on the website at enrollment. All services require appointment scheduling and provider approval.
2.2 Rollover Policy
To offer flexibility while maintaining fairness and operational practicality:
– Unused services for a given month may be carried over for **one (1) additional month only**.
– No service may roll over for more than two consecutive billing cycles (the original month + one rollover month).
– Only **one month of benefits may be active at any given time**. Members may not accumulate, stack, bank, or store multiple unused months.
– Expired services hold **no monetary value** and cannot be refunded, converted, or substituted.
2.3 Discount Eligibility
Members in good standing receive exclusive pricing and discounts on selected services. Discount eligibility requires:
– All payments are current.
– No outstanding balances.
– Compliance with all policies and medical requirements.
2.4 Exclusions
Discounts do not apply to:
– Retail products unless specifically designated.
– Promotional, special-event, or seasonal packages.
– Services already discounted.
– Services GLOWDOCS Medspa does not provide (e.g., laser hair removal, skin tightening).
3. APPOINTMENTS, SCHEDULING, AND SERVICE USAGE
3.1 Scheduling
Members are responsible for scheduling appointments in advance. Availability is not guaranteed and may vary by day, time, or provider.
3.2 Medical Evaluation
All services require a medical professional’s approval. Providers may:
– Modify the treatment plan.
– Decline treatment for safety reasons.
– Require alternative services based on clinical judgment.
3.3 NonShow & Late Cancellation Policy
Members must follow GLOWDOCS Medspa’s scheduling, cancellation, and non-show policies. Failure to attend appointments or cancel within the designated timeframe may result in:
– Forfeiture of that month’s service.
– Fees per office policy.
– Possible suspension of membership privileges for repeated offenses.
3.4 Service Expiration
All treatments, units, and credits must be used within the permitted timeframe, as outlined under the Rollover Policy.
4. TERM, MINIMUM COMMITMENT & CANCELLATION
4.1 Mandatory Minimum Commitment
Membership requires a **four (4) month minimum** billing cycle commitment. This commitment ensures proper onboarding, consistent treatment planning, and service continuity.
4.2 Early Cancellation Requests
Requests made within the first four months will be honored effective **only after** the fourth billing cycle is completed. Membership continues through the paid period.
4.3 Cancellation After Minimum Period
Following completion of the four required months, Membership automatically transitions to a month-to-month plan. Members may cancel at any time by notifying GLOWDOCS Medspa before the next billing date.
4.4 No Refunds or Prorated Credits
– No partial refunds will be issued for cancellations after billing.
– Membership continues through the end of the paid month.
– Unused services at time of cancellation expire automatically and cannot be refunded or converted.
4.5 Termination by GLOWDOCS Medspa
GLOWDOCS Medspa may terminate a Membership for:
– Non-payment
– Repeated no-shows or disruptive behavior
– Policy violations
– Medical contraindications
– Fraud or misuse of benefits
5. PAYMENT AUTHORIZATION & BILLING POLICIES
5.1 Automatic Billing
By enrolling, the Member authorizes recurring monthly charges to the saved payment method for:
– The activation fee,
– Monthly membership fees,
– Additional services or products approved at time of treatment.
5.2 Failed Payments
If a payment fails:
– GLOWDOCS Medspa may suspend benefits immediately.
– The system may reattempt payment.
– The Member is responsible for updating payment information promptly.
5.3 Updating Payment Information
Members must maintain an active, valid card on file at all times to avoid interruption of services and benefits.
6. NON-TRANSFERABILITY & NON-SHARING OF BENEFITS
Memberships are personal accounts and may not be:
– Sold
– Shared
– Gifted
– Transferred
Benefits apply only to the individual Member listed on the account.
7. CHANGES TO MEMBERSHIP PROGRAM, BENEFITS & FEES
GLOWDOCS Medspa reserves the right to update or modify:
– Pricing
– Benefits
– Terms & Conditions
– Services included or excluded
Members will be notified of major changes. Continued Membership and use of services after any update constitutes acceptance of the revised terms.
8. MEDICAL NATURE OF SERVICES & ASSUMPTION OF RISK
8.1 Medical Judgment
All cosmetic medical treatments are performed only if deemed safe and appropriate. Providers may refuse treatment for any clinical reason.
8.2 No Guarantees
Outcomes vary; no result, response, or longevity of treatment effect is guaranteed.
8.3 Member Responsibilities
Members must:
– Provide accurate medical history.
– Disclose any changes in medications or health conditions.
– Follow all pre- and post-care instructions.
9. LIMITATION OF LIABILITY
To the fullest extent permitted by law:
– GLOWDOCS Medspa’s liability is limited to the total Membership fees paid by the Member in the **three (3) months** immediately preceding the claim.
– GLOWDOCS Medspa is not liable for indirect, incidental, consequential, or special damages.
– Members assume all risks associated with elective cosmetic procedures.
10. GOVERNING LAW & DISPUTE RESOLUTION
These Terms & Conditions are governed by New York State law. Any disputes shall be resolved in the courts of Nassau County, unless otherwise legally required. Members agree to attempt informal resolution before initiating litigation.
11. SEVERABILITY
If any provision of these Terms is found unenforceable:
– The remaining provisions remain valid.
– The invalid section will be adjusted only as necessary to make it enforceable while preserving intent.
12. ENTIRE AGREEMENT
This document, together with:
– The Membership enrollment form,
– Medical consent forms,
– Office policies,
– Website Membership description,
constitutes the **entire integrated agreement** between the Member and GLOWDOCS Medspa. It supersedes all previous discussions, emails, advertisements, representations, or agreements — whether written or oral. No employee or representative may alter the Terms unless changes are provided in writing and approved by GLOWDOCS Medspa leadership. Member acknowledges that they have not relied on any statement, promise, or representation outside of these written Terms.
